Claim Check Lists

We've developed these simple check lists to help you collect the information you need to file a claim.

Auto Claim

Contact the police as soon as the accident occurs.

Record the following information before leaving the scene:

  • Names & contact information of all parties involved
  • Insurance company & policy number of all parties involved
  • Make, Model, color and license plate of all vehicles involved
  • Name of officer responding and a report number if available.
  • Record the names and contact information of any witnesses.

Property Claim

Be prepared! This often makes it easier to deal with property claims such as fire, theft, and flood. Keep an inventory of your personal property including serial numbers and approximate value if applicable which will help to determine the value of your loss.

  • Contact local authorities: police or fire services if required.
  • If possible, take steps to prevent further damage from occurring.
  • If possible, take photos or a video of the scene to help with assessment.
  • Do not discard or take steps to repair damaged property without approval from your insurance representative.

Liability Claim

  • Contact local authorities: Police, emergency personnel, etc. if required.
  • Record the names & contact information of each person involved.
  • Identify any hazards in the accident area.
  • Retain all items related to the incident.
  • Record the names of any authorities investigating the scene.
